Visiting Nurse Service has created the Care of the Heart program to help patients understand and care for their disease. Under this innovative program, the patient and a VNS nurse work together to improve the patient's condition and quality of life.
When a patient known to have heart disease experiences shortness
of breath or another symptom requiring prompt medical attention,

The patient contacts his visiting nurse partner, who visits within two hours.

Treatment prescribed by the patient's cardiologist is administered by the visiting nurse, and a visit to the emergency room, and even hospitalization, is avoided.

The patient learns about her disease and how to best manage it, with the help of medical professionals.
