How to Apply: Residency Application Materials

 

PGY-1 Pharmacy Practice Residency Program

Requirements for Admission

Admission to the residency program is available to Pharm.D. graduates of accredited colleges of pharmacy.  Residents must be a United States citizen or carry a permanent resident visa.  Residents must also be eligible for New York State licensure.

Application Process

An application form is not required, but the following items must be submitted to apply for the residency:

  1. Letter of intent stating goals and interests
  2. Curriculum vitae
  3. Three letters of reference, utilizing the ASHP residency reference form
  4. Official college transcripts

The deadline for the receipt of application materials to be considered for an onsite interview is January 15, 2008. A formal presentation is required during the interview.

ASHP Match

The Pharmacy Practice Residency at the University of Rochester Medical Center is an ASHP accredited residency and utilizes the Resident Matching Program.  In compliance with the rules and regulations of the National Matching Services and ASHP, the residency site agrees that no person will solicit, accept, or use any ranking-related information from any residency applicant.

PGY-2 Infectious Diseases Pharmacy Practice Residency Program

Admission to the residency program is available to Pharm.D. graduates of accredited colleges of pharmacy who have completed a pharmacy practice (PGY-1) residency.  Applicants must be a United States citizen or carry a permanent resident visa.  Applicants must be eligible for New York State licensure.

To apply for the residency, the following items must be submitted:

  • Letter of intent stating goals and interests
  • Curriculum vitae
  • Three letters of reference on the ASHP Recommendation Form
  • Official college transcripts

Application materials must be received by January 15, 2008 to be considered for an onsite interview.  A formal presentation is required during the interview.
